creat Floating Ad in EA with Close button

I want to create a Floating Ad in EA; size 300x250 px. We have been successful with the replay button and adding a click Tag. Can anyone give me a template that I can open and use in EA and then publish to Google DoubleClick media. the Close button will be a small 'X' mark in the corner.
