Add a single form box into Edge Animate

I need to add a single form box for a user to type a message into, and then have their message vanish when the user clicks a button next to the form (by form I just mean a little box they can type into). How do I add this code? It is for a game, so it doesn't matter what they type into the form, it just has to vanish when the submit button is clicked.
